Per the Illinois Uniform Peace Officers’ Disciplinary Act, 50 ILCS 725, a person filing a complaint that is made against a sworn officer must use this sworn affidavit form and have it notarized:

The Cook County Sheriff’s Office serves at the Inspector General for the following Cook County municipalities. We investigate complaints of possible theft, fraud, waste, misconduct, and mismanagement by elected officials and employees of taxing districts within the county, especially those affiliated with suburban municipal governments. The Sheriff’s Office then submits reports with their findings to the responsible elected officials of the governing agency, and where appropriate, makes referrals for criminal charges to prosecuting authorities such as the State's Attorney's Office or Attorney General’s office.
